To change your personal data you must fill i the form to change personal data found on our website under documents and return it to info@mijnsocialehygiene.nl. We also ask for a copy of the identity document. You can protect your photo and social security number if you wish. When we receive the documents (preferably by e-mail) we will process them. You will always receive an email from us with whether the data has indeed been changed or whether it will not be changed due to too many inaccuracies.
